By checking, I understand and agree that due to the time allotted for treatments, I understand that a $90 security deposit is required to secure your appointment. My final cost may be greater if additional products and/or services are purchased. 

(Appointments scheduled either for Weekends, or for Weekdays before 8 AM and after 5:30 PM, are subject to a $90 surcharge.)

If I fail to keep my scheduled appointment without rescheduling, my security deposit will be retained as a cancellation fee. We understand things do come up and we simply ask for the courtesy of a phone call in advance.
